Surgical treatment for a patient with long-standing persistent atrial fibrillation and severe mitral insufficiency following myocarditis is difficult but necessary to improve quality of life.
Why the study?
Structural and functional cardiac remodeling causing arrhythmias and heart failure poses a major challenge, where extensive cardiac surgery is difficult but conservative management carries poor quality of life and high cardiovascular risk.

Surgical treatment for severe structural and functional cardiac remodeling following myocarditis is complex but may be necessary to improve quality of life and reduce adverse cardiovascular events.
The relationship between atrial fibrillation and thromboembolic syndrome and, above all, ischemic stroke is well known. Atrial cardiomyopathy is important for understanding the mechanisms of thrombogenicity. In this report, we consider diagnostic algorithm, surgical treatment of a patient with long-standing persistent atrial fibrillation, thromboembolic complications, severe mitral insufficiency and atriomegaly following myocarditis. Structural and functional cardiac remodeling with subsequent arrhythmias and heart failure is a serious problem in modern cardiology and cardiac surgery. Extensive cardiac surgery is extremely difficult and does not always lead to significant clinical improvement. However, quality of life is quite low without surgical treatment in such patients and associated with high risk of adverse cardiovascular events.

Fedotkina et al. (2024) conducted a case report in Atrial fibrillation, thromboembolic complications, severe mitral insufficiency, atriomegaly following myocarditis (n=1). Surgical treatment was evaluated. Surgical treatment for a patient with long-standing persistent atrial fibrillation and severe mitral insufficiency following myocarditis is difficult but necessary to improve quality of life.
