Aorto–left ventricular tunnel (ALVT) is an uncommon congenital cardiac anomaly characterized by an abnormal extracardiac communication between the ascending aorta and the left ventricle (LV).1,2 Prenatal diagnosis is rare and challenging because findings may mimic primary aortic regurgitation (AR) or coronary artery fistula.3,4 Advanced volumetric techniques such as spatiotemporal image correlation (STIC) may enhance anatomical understanding and diagnostic confidence.5-7 Early recognition of ALVT is critical, as it may present with severe fetal AR and significant hemodynamic consequences.
