A noninvasive electrophysiology study in 1 patient with permanent His bundle pacing demonstrated that intermittent left bundle recruitment was caused by postrepolarization refractoriness.
Case Report (n=1)
The underlying mechanism of bundle branch block, such as postrepolarization refractoriness, has important implications for patient selection and device programming in His bundle pacing.
Hypothesis-generating for postrepolarization refractoriness in His bundle pacing; leaves open effects on patient selection and programming.
We present a case of a patient with elevated His lead capture thresholds and intermittent recruitment of the left bundle. The patient underwent a noninvasive electrophysiology study and was determined to have a left bundle branch block due to postrepolarization refractoriness. The nature of bundle branch block can have important implications for optimal patient selection and device programming in the emerging field of His bundle pacing.

Friedman et al. (2016) conducted a case report in Left bundle branch block with elevated His lead capture thresholds (n=1). Permanent His bundle pacing and noninvasive electrophysiology study was evaluated. A noninvasive electrophysiology study in 1 patient with permanent His bundle pacing demonstrated that intermittent left bundle recruitment was caused by postrepolarization refractoriness.
