Key Points The trajectory of severe cognitive impairment following intensive care in the oldest‐old remains poorly quantified. Among previously independent nonagenarian sepsis survivors, 4.1% were newly prescribed anti‐dementia medications within 6 months post‐ICU. This initiation suggests either an accelerated dementia, the unmasking of previously undiagnosed dementia, or a “prescribing cascade.” Comprehensive post‐ICU care should include cognitive screening tailored for our oldest, most vulnerable survivors.
