A questionnaire was developed as a research or diagnostic tool for needs assessment of the nutritional knowledge and eating behaviors of older adults. The questionnaire consists of 6 sections: demographic and personal information, food resources, food consumption patterns, dietary practices related to health, activity patterns, and nutritional knowledge. The results of a study involving noninstitutionalized seniors demonstrated the questionnaire's usefulness for nutritional needs assessments.
