Surgical approaches in the treatment of complications of CPAP therapy in pediatric otorhinolaryngology

Abstract

Material and methods. A retrospective analysis of the treatment results of 7 patients with nasal columella defect after CPAP therapy with binasal cannulas, who underwent reconstructive stages using free revascularized flaps, was performed. All patients were treated at the Department of Pediatric Otorhinolaryngology at the Federal State Budgetary Institution NMICO of the FMBA of Russia in the period from 2015 to May 2025. Results. The minimum follow-up period is from 6 months to 4 years. 90% of the patients were invited for a follow-up after 1 year to determine the tactics of further treatment. During the examination, positive anatomical and aesthetic results were observed in dynamics, improving the quality of life of patients. Conclusions. The restoration of the nasal columella after the application of CPAP therapy with binasal cannulas according to our proposed method has a good result when monitoring patients in dynamics, and can be used in the department of pediatric otorhinolaryngology. Цель исследования: оценка эффективности хирургического лечения пациентов с дефектом колумеллы носа после применения CPAP (Continuous Positive Airway Pressure)-терапии. Материал и методы. Был проведен ретроспективный анализ результатов лечения 7 пациентов с дефектом колумеллы носа после применения CPAP-терапии биназальными канюлями, которым были выполнены реконструктивные этапы с использованием свободных реваскуляризированных лоскутов. Все пациенты проходили лечение на базе отделения детской оториноларингологии в ФГБУ НМИЦО ФМБА России в период с 2015 по май 2025 г. Результаты. Минимальный срок наблюдения за пациентами составлял от 6 месяцев до 4 лет, 90% пациентов были приглашены на повторный осмотр через 1 год для определения тактики дальнейшего лечения. При осмотре в динамике наблюдали положительные анатомический и эстетический результаты, улучшающие качество жизни пациентов. Заключение. Восстановление колумеллы носа после применения CPAP-терапии биназальными канюлями по предлагаемой нами методике имеет хороший результат при наблюдении пациентов в динамике, может использоваться в отделении детской оториноларингологии.
