Cremation tattooing—the incorporation of a loved one’s cremated remains into tattoo ink—is an emerging memorial practice in Canada that occurs outside existing cosmetic ink regulations. This article examines the risks and considerations associated with this practice. Adding cremains to ink may introduce uncharacterized metals, inorganic particulates, and potential contaminants, altering ink chemistry and increasing risks of inflammation, infection, or pigment instability. Post-cremation handling can reintroduce microbes, while ashes from individuals treated with radiopharmaceuticals may contain residual radionuclides requiring case-specific precautions. Physical challenges, including inconsistent particle size and changes in viscosity, can affect equipment function, healing, and pigment retention. Despite growing cultural interest, no Canadian jurisdiction provides specific guidance, leaving practitioners and consumers without evidence-based standards. This article highlights major knowledge and regulatory gaps and offers recommendations to support risk communication and safer practice. Further research is urgently needed to understand the health implications of cremation tattoos.
