Epiphyseal plate injury in the dog: a radiographic study of growth disturbance in the forelimb

ABSTRACT Radiographic studies of thirty‐five dogs with growth disturbance in the forelimb were reviewed. There were nine giant and large breeds and five smaller breeds included in the material. The mean age at the time of injury was 4 months; angular deformity, when it occurred, was seen 5–7 weeks after the injury. Twenty‐one of the dogs had radiographs taken immediately following injury and a fracture line could be identified in sixteen. Premature closure of the epiphyseal plates was seen involving the distal ulna in twenty‐six dogs, the distal radius in four dogs, both the distal ulna and radius in three dogs, and the proximal radius in two dogs. The radio‐graphic findings associated with epiphyseal plate closure in each of these anatomical locations are discussed. Radiography is required in the diagnosis and treatment of growth abnormalities of the radius and ulna in the dog. The radiographic evaluation should include a detailed study of the epiphyseal plates and adjacent metaphyses of both forelimbs. This study should determine (1) the lengths of the radii and ulnas; (2) the width of the joint spaces at either end of the shortened bone; (3) direction of the diaphyseal bowing;(4) the angulation in the carpus; and (5) the extent of arthritic change in the elbow and carpus. These factors will aid in reaching a diagnosis as described in this paper. The differential diagnosis of elbow lameness and radial‐carpal lameness and/or angulation is discussed. Résumé. On présente des radiographies de 35 chiens atteints de troubles de croissance des inembres antérieurs. Il s'agissait de 9 races géantes ou grandes et de 5 races plus petites. L'âge moyen au moment de l'accident était de 4 mois et la déformation angulaire lorsqu'elle existait, apparaissait 5 à 7 semaines après le traumatisme. Dans 21 cas, on a pris des radiographies immé‐diatement après ce dernier et on a pu identifier une lignede fracture dans 16 cas. Une ossification prématurée des plateaux épiphysaires intéressait le cubitus distal chez 26 chiens, le radius distal dans 4 cas et le cubitus et le radius distaux dans 3 cas, enfin le radius proximal chez 2 animaux. Les constatations radiologiques dans le cas d'ossification des plateaux épiphysaire ont été discutées pour chacune de ces localisations. La radiographie est nécessaire pour le diagnostic et le traitement des anomalies de croissance du radius et du cubitus du chien. Une évaluation à l'aide des rayons X devrait être pratiquée chaque fois que l'on étudie les plateaux épiphysaires et les métaphyses adjacentes des deux membres antérieurs. Cette étude doit déterminer: (1) la longueur des radius et des cubitus; (2) l'épaisseur de l'espace interarticulaire à chaque extrémité de l'os raccourci; (3) la direction de l'incurvation diaphysaire; (4) l'angulation du carpe; et (5) l'étendue des lésions arthritiques du coude et du carpe. Tous ces facteurs facilitent le diagnostic dans les cas decrits dans le présent article. Le diagnostic différentiel des atteintes du coude et de l'articulation radio‐carpienne ainsi que des angulations associées ou non à ces lésions est discuté. Zusammenfassung. Radiographische Untersuchungen von 35 Hunden mit Wachstums‐störungen der Vorderbeine werden besprochen. Es handelte sich um neun sehr grosse und grosse Rassen sowie um fünf kleinere. Das mittlere Alter zur Zeit der Verletzung betrug 4 Monate; winklige Deformität wurde vorkommendenfalls 5–7 Wochen nach der Verletzung beob‐achtet. Von 21 dieser Hunde wurden Röntgenaufnahmen unmittelbar nach der Verletzung angefertigt, und in 16 Fällen wurde eine Bruchlinie gefunden. Vorzeitige Schliessung der Epiphysealplatten wurde bei der distalen Ulna bei 26 Hunden beobachtet, beim distalen Radius bei 4 Hunden, beim distalen Radius und der distalen Ulna bei 3 Hunden und beim proxi‐malen Radius bei 2 Hunden. Die mit Epiphysealplattenverschluss verbundenen Röntgenbefunde für jede dieser anatomischen Lokalisationen werden hesprochen. Die Röntgenuntersuchung ist für die Diagnose und die Behandlung von Wachstumsanomalitäten des Radius und der Ulna beim Hund erforderlich. Diese Röntgenuntersuchung muss auch eine eingehende Untersuchung der Epiphysealplatten und benachbarten Metaphysen beider Vorderbeine umfassen. Bei dieser Untersuchung sind zu bestimmen (1) die Länge der Radii und Ulnae, (2) die Breite der Gelenkabstände an jedem Ende des verkürzten Knochens, (3) die Richtung der diaphysealen Biegung, (4) die Krümmung im Carpus und (5) das Ausmass der arthritischen Veränderung im Ellbogen und Carpus. Diese Faktoren tragen zur Diagnosestellung wie in dieser Abhandlung beschrieben bei. Die Differentialdiagnose der Ellbogenlahmheit und der Radialcarpallahmheit und bzw. oder ‐krümmung wird besprochen.
