ALK-positive anaplastic T cell lymphoma (ALCL) is more common in children than in adults and more common in males than in females 1. Leukemic manifestations, as seen in this patient, are more common in the small cell variant 1. Hemophagocytic lymphohistiocytosis is a life-threatening disorder that can occur in a familial primary form or be a response to infections, rheumatological disorders or neoplasms, particularly T-lineage lymphomas. It has occasionally been reported in association with ALCL.
