BACKGROUND: Maxillary sinus floor elevation (MSFE) requires reliable graft materials to ensure adequate bone regeneration. Bio-Oss Collagen and Bio-Oss granules are widely used xenografts, but evidence comparing their osteogenic performance remains limited. OBJECTIVE: This study evaluated whether collagen addition influences early bone height gain or trabecular bone quality after MSFE. METHODS: Twenty-four patients undergoing conventional lateral window osteotomy for sinus floor elevation with simultaneous implant placement were retrospectively analyzed. According to graft type, cases were assigned to the Bio-Oss Collagen (n=12) or Bio-Oss granules (n=12) group. Radiographic parameters and the quality of newly formed bone were assessed with cone-beam computed tomography (CBCT) and through trabecular bone parameter analysis using CTAn software (CTAnalyzer, SkyScan, Antwerp, Belgium), respectively. RESULTS: After 6 months of healing, no significant differences in bone height were observed between the groups (P>0.05). Similarly, trabecular parameters also showed no statistically significant variation (P>0.05). Regression analysis further indicated that the type of graft material was not a predictor of early bone volume or microarchitectural maturation. CONCLUSIONS: Bio-Oss Collagen and Bio-Oss granules demonstrated comparable early osteogenic outcomes following MSFE, with no measurable differences in bone height gain or trabecular microarchitecture. The incorporation of collagen did not appear to enhance early bone regeneration.
