Introduction and Objective: Recent trends in antiglycemic pharmacotherapy among older adults have not been studied. Methods: We conducted a repeated cross-sectional study annually from 2017-2024 in Epic Cosmos, a nationwide database of electronic health records of more than 300 million patients. Patients age =65 with a history of diabetes mellitus were identified via a validated algorithm. Rates of new prescription (in each index year) and prevalent prescription (in the two years preceding each index year) of the six most common antiglycemic drug classes were estimated annually and adjusted for age and sex. Results: Between 2,391,060 (in 2017) and 6,204,836 (in 2024) patients were included. Trends in prevalent prescriptions are shown in Figure 1 and trends in new prescriptions are shown in Figure 2.
