This document presents INOMED’s contribution to the European Commission Call for Evidence on the future Council Recommendation on cardiovascular health checks under the Safe Hearts Plan. Drawing on implementation experience from the Leresti Living Lab in Romania and European personalised prevention initiatives, the paper proposes a framework for transforming cardiovascular health checks from isolated screening activities into the starting point of a personalised prevention journey. The recommendations emphasise earlier risk identification, equitable access to prevention, continuity of care and long-term follow-up. Key proposals include the establishment of a minimum European core package for cardiovascular health checks, the integration of Lp(a) testing in personalised cardiovascular risk assessment, the use of validated risk prediction tools (including AI-enabled tools where appropriate), the recognition of Living Labs and social innovation as implementation enablers, and the development of structured referral and follow-up pathways. The document also highlights the role of digital health solutions in reducing rural-urban inequalities, improving prevention uptake and supporting continuity of care, while respecting subsidiarity and Member States’ responsibility for organising healthcare services. INOMED argues that the future Council Recommendation represents an opportunity to move from fragmented cardiovascular screening towards personalised, equitable and actionable prevention across Europe, contributing to the Safe Hearts Plan objective of reducing premature cardiovascular mortality by 25% by 2035.
