The aim was to identify cephalometric factors associated with the initiation of orthodontic treatment.
Retrospective observational study using clinical records from a private orthodontic center in Poland.
Included 902 patients aged ≥ 11 years with lateral cephalograms between 2022 and 2024.
Conducted logistic regression analyses to assess associations between cephalometric parameters and treatment initiation.
490 out of 902 participants initiated orthodontic treatment.
Increased NSL–NL angle was associated with a lower likelihood of beginning treatment, while greater maxillary incisor protrusion (1+:NA, mm) increased the likelihood (OR = 1.053).
Most cephalometric variables showed no significant relationship with treatment initiation, indicating selective associations predominantly with vertical skeletal configuration and maxillary incisor position.
