Shock heart team-guided emergency SAVR with VA ECMO enables survival in severe aortic cardiogenic shock.
Case Report (n=1)
No
Highlights the feasibility and importance of a multidisciplinary heart team approach for managing cardiogenic shock and critical mixed valvular disease using salvage surgery and ECMO.
Supports heart team-guided emergency SAVR with VA ECMO in select mixed aortic valve cardiogenic shock cases; leaves open generalizability and optimal patient selection.
A 65-year-old male was admitted for cardiogenic shock (ejection fraction: 15%) with severe aortic stenosis and regurgitation. He underwent emergency angiography and mechanical circulatory support. A multidisciplinary heart team discussed treatment options. Ultimately, he underwent successful emergency surgical aortic valve replacement with recovery on extracorporeal membrane oxygenation. (Level of Difficulty: Intermediate.)

Haneke et al. (2023) conducted a case report in Cardiogenic shock with severe aortic stenosis and regurgitation (n=1). Emergency surgical aortic valve replacement and VA ECMO was evaluated on Survival to hospital discharge. Emergency surgical aortic valve replacement with recovery on VA ECMO, guided by a multidisciplinary shock heart team, resulted in successful recovery and hospital discharge in a 65-year-old male with cardiogenic shock and mixed severe aortic valve disease.
