Acute type B aortic dissection progresses to type A within ~3 minutes despite stable hemodynamics.
Why the study?
Acute Stanford type B aortic dissection is typically managed conservatively in hemodynamically stable patients, but proximal extension to the ascending aorta may occur unpredictably.
Case Report (n=1)
Highlights that acute Stanford type B aortic dissection can rapidly progress to type A within minutes without hemodynamic deterioration, emphasizing a critical limitation of single-phase CTA imaging.
Rapid type B-to-A progression can occur in minutes without instability; case report leaves open optimal CTA protocols for acute aortic syndromes.
BACKGROUND: Acute Stanford type B aortic dissection is generally managed conservatively in hemodynamically stable patients. However, proximal extension to the ascending aorta may occur unpredictably. CASE SUMMARY: An 84-year-old woman presented with isolated chest pain and stable hemodynamics. Initial computed tomography angiography (CTA) suggested acute Stanford type B aortic dissection extending to the aortic bifurcation. During the same CTA examination, delayed-phase imaging obtained 30 seconds later demonstrated new extension into the ascending aorta. Repeat CTA performed 3 minutes after the initial scan showed further proximal progression, confirming acute Stanford type A dissection. Emergent total arch replacement with open stent grafting was performed, and the postoperative course was uneventful. DISCUSSION: This case demonstrates that acute Stanford type B aortic dissection may progress to type A within minutes, even without hemodynamic deterioration, highlighting an important limitation of single-phase imaging and a cautionary message when conservative management is considered.

Yokawa et al. (2026) conducted a case report in Acute Stanford type B aortic dissection (n=1). Serial computed tomography angiography (CTA) was evaluated on Progression to Stanford type A aortic dissection. Acute Stanford type B aortic dissection progressed to type A within 3 minutes during a single CTA examination despite stable hemodynamics.
