Case report reveals hidradenitis suppurativa as a complication from nirogacestat treatment for desmoid tumors, suggesting the need for monitoring.
Desmoid tumors are locally aggressive benign neoplasms. Nirogacestat was FDA approved in 2023 as a targeted therapy for desmoid tumors. It is a gamma-secretase inhibitor that blocks Notch signaling and has shown substantial value in desmoid tumor treatment. Though highly effective, nirogacestat has many side effects, including hidradenitis suppurativa (HS) that presents with painful cutaneous nodules and abscesses. We present a case of desmoid tumor, treated with nirogacestat and then developed HS. Though HS could be an incidental finding on 18 F FDG PET/CT, PET can assess the extent and severity of lesions.
