Case report identifies Toscana virus causing meningitis in a UK traveller returning from Spain, highlighting under-reported infections.
Toscana virus is a well-recognised cause of meningitis around the Mediterranean basin in the warmer summer months. The incidence is likely under-reported as the spectrum of disease ranges from asymptomatic to an aseptic meningo-encephalitis. In Italy from 2018 to 2020, where the virus is endemic, there were 60 reported cases of neuro-invasive infection. The usual clinical presentation is with a febrile illness, headache, fever, and vomiting; treatment is usually supportive. We report a case of neuro-invasive Toscana virus infection diagnosed in the United Kingdom by PCR of Toscana virus from the cerebro-spinal fluid of a traveller returning from Southern Spain.
