Abstract Electronic cigarettes (e-cigarettes) have been increasingly promoted as safer alternatives to conventional smoking and as smoking cessation tools. However, emerging evidence has linked e-cigarette use to serious pulmonary complications, including e-cigarette or vaping product associated lung injury (EVALI). Severe cases may progress to acute respiratory distress syndrome (ARDS), requiring advanced respiratory support. We report the case of a previously healthy 25-year-old female who developed severe vaping-associated ARDS refractory to conventional mechanical ventilation and was successfully managed with veno-venous extracorporeal membrane oxygenation (VV-ECMO). This case highlights the potential severity of vaping-related lung injury and emphasizes the role of ECMO as a life-saving intervention in selected patients.
