Background & Objective: Vitamin E is a fat-soluble vitamin with influential antioxidant features that has also been linked to better regulation of the hormonal system and helps egg maturation. The present study was conducted with aim to assess the role of vitamin E in enhancing female fertility.Materials & Methods: This randomized clinical trial was conducted in 2024-2025 on 112 patients diagnosed with unexplained infertility who were divided into 2 groups vitamin E supplementation (group A) in addition to controlled ovarian stimulation. and without vitamin E supplementation (group B) who received controlled ovarian stimulation only. Data were analyzed by SPSS software (version 26). P≤0.05 was considered statistically significant, to assess and compare the effect of vitamin E supplementation in both groups. Results: Endometrial thickness in the group treated with Vit E was 9.8±2.3 mm while in those without Vit E was 8.3±2.6 mm. The number of mature follicles was also increased in the group treated with Vit E (2.4±1.1) than those without Vit E (1.6±1.0). Despite increases in the implantation rate and ongoing pregnancy in the group of patients treated by Vit E than those without Vit E, there are no significant differences between the two groups (P>0.05).Conclusion: Vitamin E supplementation can improve endometrial receptivity by significant increment in endometrial thickness. Although implantation and successful pregnancy rates were higher in the vitamin E group, these differences were not statistically significant. Further larger studies are needed to confirm the potential reproductive benefit of vitamin E.
