The development of smart and digital wearable technologies has transformed conventional healthcare approaches by facilitating real-time and continuous health tracking and evidence-based clinical decisions. This review summarizes the latest innovations in smart wearable technologies in the medical and lifestyle fields and how they contribute to personalized healthcare through continuous monitoring, behavioral insights, and clinical decisions. Web of Science, Scopus, Google Scholar, and relevant gray literature sources were used to retrieve and critically analyze peer-reviewed publications. Findings from epidemiological and design-based studies demonstrate improved disease monitoring, rehabilitation, preventive care, and better management of patients in various population groups. Applications of lifestyle wearable devices are briefly discussed to illustrate how behavior and lifestyle influence health; however, the review is mainly concerned with the clinical implications, technological limitations, regulatory requirements, and degree of recent patenting activity of the technology. Although smart wearables have limitations, such as privacy of data, power utilization, cost effectiveness, and scalability, they have the potential to revolutionize personal healthcare. Overall, intelligent wearable devices have significant potential for improving sustainable personalized healthcare when accompanied by regulatory and ethical management of data.
