As populations age, the long-term cumulative burden of lifestyle-related diseases has become a major determinant of age-related morbidity, with hypertension playing a central role. Sustained vascular stress due to inadequately controlled blood pressure accelerates cardiovascular, cerebrovascular, and renal disease progression and contributes to systemic target organ damage. Despite the critical importance of effective blood pressure control for extending healthy life expectancy, hypertension control rates in Japan remain suboptimal. Contributing factors include clinical inertia, low adherence, and difficulties in selecting appropriate antihypertensive therapies. Hypertension is a heterogeneous condition influenced by lifestyle, pathophysiology, and genetic background, making cause-specific treatment difficult in routine clinical practice. Consequently, pharmacotherapies with multifaceted mechanisms of action continue to be essential. In recent years, major breakthroughs in antihypertensive and cardiometabolic therapeutics have expanded the treatment paradigm beyond blood pressure reduction alone. Novel agents and established therapies have demonstrated pleiotropic benefits, including cardiovascular and renal protection and improved metabolic profiles. This review summarizes recent advances in the treatment of hypertension-related disease, with a focus on emerging pharmacotherapies such as zilebesiran, aldosterone synthase inhibitors and on established or repurposed agents including mineralocorticoid receptor antagonists, angiotensin receptor–neprilysin inhibitors, GLP-1 receptor agonists, a dual GIP/GLP-1 receptor agonist, and SGLT2 inhibitors. We discuss their clinical implications, particularly in relation to adherence, pathophysiological heterogeneity, and the future of personalized hypertension management aimed at improving long-term outcomes.
