This guide discusses the identification and treatment of J-pouch complications to enhance patient quality of life.
The Ileo Pouch Anal Anastomosis J-pouch is currently the standard of care for patients with ulcerative colitis undergoing total colectomy and who are candidates for restorative surgery. Surgical techniques and the management of this type of patient in referral centers have improved over time. Promptly and correctly identifying and treating complications from this type of surgery is essential to ensuring proper pouch function in a very young surgical population. These complications can sometimes be treated conservatively and medically. Cases requiring resurgical intervention must be thoroughly evaluated. Pouch revision surgery is a very delicate procedure, both technically and psychologically, and should be performed exclusively in referral centers. However, the recognition and management principles of pouch complications must be the prerogative of all general surgeons with the aim of preserving the functionality and quality of life of these patients.
