心臓再同期療法(以下,CRT)において冠状静脈(以下,CV)の解剖を術前に把握しておくことは重要である.われわれは以前より単純CTでCVを直接同定し,手動で描出を行っているが,CV分枝の描出は困難なことも多い.そこで,心外膜脂肪(以下,Fat)とCVの関係に着目しFatを描出することでCVの解剖学的評価ができる新しい方法(以下,Fat描出法)を考案し,その正確性について評価を行った.CRT植込みを行った52人に対し,単純CTからCV本幹とFat描出法によりCV分枝を描出した.描出されたCV分枝は部位別に前壁,側壁,後壁と3つに分類し,植込み術中に行われたCV造影と比較し,正確性を3段階(grade1:末梢まで描出,grade2:途中まで描出,grade3:描出不能)で評価した.植込み対象となるCV分枝は前壁で21本,側壁で26本,後壁23本の計70本.前壁ではgrade1が8本(38.1%),grade2が5本(23.8%),grade3が8本(38.1%),側壁ではgrade1が14本(53.8%),grade2が4本(15.4%),grade3が8本(30.8%).後壁ではgrade1が15本(65.2%),grade2が6本(26.1%),grade3が2本(8.7%)だった.Fat描出法はFatの存在に依存するが,CV分枝を正確に描出可能である.
