Abstract This technical note describes a single‐incision, onlay, double all‐suture anchor technique for distal biceps tendon repair. The method enables anatomic reattachment of the tendon onto the ulnar aspect of the radial tuberosity, optimizing footprint restoration and tendon‐bone contact area compared with conventional single‐incision techniques. This configuration provides secure fixation that permits immediate postoperative motion and offers a secondary anchor for added stability should one anchor fail.
