Abstract Prostate carcinoma is considered a heterogeneous disease, often creating challenges for diagnosis and treatment. Tumor heterogeneity within a single patient is rarely assessed using molecular imaging. Furthermore, aggressive variants of prostate cancer have shown resistance to conventional androgen deprivation therapy. In this case report, we present a patient with an aggressive form of metastatic prostatic adenocarcinoma, exhibiting heterogeneity as revealed by prostate-specific membrane antigen and fluorodeoxyglucose positron emission tomography imaging, and treated with an innovative combination of cabazitaxel and carboplatin, resulting in a positive clinical, serological, and radiological response. However, these findings are derived from a single case and should be interpreted as hypothesis-generating.
