A case of chronic ocular ischemia secondary to occlusive vascular disease of the carotid artery is presented. The clinical presentation and method of diagnosis are discussed. Early recognition of this syndrome is important because an extracranial-intracranial artery bypass procedure can reverse chronic ischemia. (Neurosurgery, 7: 480-483, 1980)
