In this case series from the British Association of Dermatologists Biologic and Immunomodulators Register (BADBIR), we evaluated serious infections among 161 children receiving systemic treatments for psoriasis between 2007 and 2024. Over a mean follow-up of approximately 2 years, 24 serious infection events occurred in 20 patients (incidence rate 31.4 per 1000 patient-years), most commonly skin and respiratory infections, with no infection-related deaths. Although hospitalisation was required in all cases, most infections were manageable, supporting the overall favourable long-term safety profile of systemic therapies in paediatric psoriasis.
