Review summarizes evidence for Z-POEM and D-POEM interventions for esophageal diverticula, indicating surgical advancements.
Esophageal diverticulae can occur in all segments of the esophagus. The majority are pulsion type diverticula, caused by a combination of luminal hyper-pressurization and adjacent muscle spasm. Symptoms can include dysphagia, regurgitation, recurrent cough and pain. Surgical intervention is indicated to relieve symptoms, but due to perioperative risk, has historically been reserved for larger, more symptomatic diverticula. Borrowing the per-oral endoscopic myotomy (POEM), a third space endoscopy techniques developed to treat achalasia, transoral endoscopic procedures have been developed to address esophageal diverticula from within submucosal tunnels. This review summarizes existing evidence for Zenker's divertula per-oral endoscopic myotomy (Z-POEM) and diverticula per-oral endoscopic myotomy (D-POEM).
