Pulsed field ablation achieves safe pulmonary vein isolation without device interaction in a pacemaker patient.
Why the study?
The safety of pulsed field ablation using a variable loop circular catheter in patients with cardiac implantable electronic devices is unexplored because such patients were excluded from clinical studies to date.
Case Report (n=1)
A novel variable loop circular pulsed field ablation catheter was safely used for pulmonary vein isolation in a patient with a dual-chamber pacemaker without causing any device interaction or malfunction.
Single-case safety with pulsed field ablation near pacemakers; hypothesis-generating and requires prospective confirmation before broader use.
Preclinical and clinical studies have demonstrated the safety and efficacy of a variable loop circular catheter (VLCC) for pulsed field ablation (PFA).1,2 There are a few studies exploring the safety of the pentaspline PFA catheter in patients with cardiac implantable electronic devices (CIEDs).3–5 The safety of the PFA VLCC in this common patient population is so far unexplored because they have been excluded from the clinical studies of the VLCC to date.

Iqbal et al. (2024) conducted a case report in Paroxysmal atrial fibrillation in a patient with a dual-chamber pacemaker (n=1). Variable loop circular pulsed field ablation (VARIPULSE) was evaluated on Safety (pacemaker malfunction or interaction) and efficacy (pulmonary vein isolation). The variable loop circular pulsed field ablation catheter was safely used for pulmonary vein isolation in a patient with an implanted dual-chamber pacemaker with no device interactions observed.
