The poultry value chain in Ghana is characterized by poor coordination, informal business models, and sustainability challenges. This study applied a mixed-methods design and a sustainable value chain framework integrated with institutional and business model perspectives to examine 268 actors in the Central Region. Data were analyzed using value chain mapping, Duncan’s dissimilarity index, and SWOT analysis. Findings indicate women dominate most chain functions, except production, while perceived sustainability varies among actors. Limited business model adoption constrains value creation and resilience. The findings highlight the need to strengthen coordination and promote business models to improve sustainability in emerging economies.
