The Michigan Society of Health-System Pharmacists (MSHP) held a Pharmacy Practice Model Initiative Michigan conference (PPMI2) in October 2011 after the ASHP Pharmacy Practice Model Summit.1,2 After the PPMI2 conference, the 2013 MSHP board of directors (BOD) identified a need to further address ambulatory care practice within the state and planned to hold a conference similar to PPMI2 in spring 2014. Given that ASHP was conducting an Ambulatory Care Summit in March 2014, the MSHP board elected to delay the Michigan conference to September 2014. The MSHP BOD organized the Ambulatory Care Summit for Michigan, inviting individuals from across the state with varying ambulatory care pharmacy practices and responsibilities. These included pharmacists, pharmacy faculty, and trainees practicing in ambulatory care clinics, transitions-of-care roles, physician organizations, and administrative roles. Based on the outcomes of the ASHP Ambulatory Care Summit, the planning group identified six areas to be explored by the invitees and identified a keynote speaker.3
