Implication

Waist circumference may not reliably reflect hepatic fat in female NAFLD patients; hypothesis-generating for sex-specific thresholds in future studies.

Structured PICO

Does waist circumference correlate with hepatic fat accumulation differently in male versus female patients with NAFLD?

P
Population
221 Japanese patients with non-alcoholic fatty liver disease (NAFLD) (103 females), selected from 2,111 consecutive patients with at least one of two criteria for liver disease (ALT >30 IU/mL and AST/ALT ratio <1). Excluded: positive for HBV, HCV, or autoimmune antibodies, and alcohol intake >20 g/day.
I
Intervention
Waist circumference and visceral fat area (VFA) measurement
O
Outcome
Correlation between hepatic fat accumulation (estimated by liver/spleen attenuation ratio [L/S ratio] on CT) and waist circumference / visceral fat area (VFA)surrogate

Waist circumference correlates with hepatic fat accumulation in male but not female Japanese NAFLD patients, suggesting waist measurement is more susceptible to gender differences than visceral fat area.
