Key result
Rescue cryoballoon ablation achieves transmural roofline block after failed radiofrequency ablation.
Why the study?
Randomized studies have failed to demonstrate significant improvement in rhythm outcomes with left atrial posterior wall isolation added to PVI alone in persistent AF, despite observational associations with reduced recurrence.
Case Report
Randomized trials show no rhythm benefit from LAPW isolation; challenges observational data and leaves optimal adjunctive ablation open.
Linear ablation for left atrial posterior wall (LAPW) isolation is a common adjunctive approach to pulmonary vein isolation (PVI) to modify an arrhythmogenic substrate and focal sources in persistent atrial fibrillation (AF). Although previous studies have reported that LAPW isolation was associated with a more significant reduction in AF recurrence compared with PVI alone,1,2 randomized studies have failed to demonstrate a significant improvement in rhythm outcomes after LAPW isolation compared with PVI alone.

Kato et al. (2025) conducted a case report in persistent atrial fibrillation. rescue cryoballoon ablation was evaluated. Rescue cryoballoon ablation was utilized to achieve transmural left atrial roofline block following unsuccessful radiofrequency ablation.
