Key result
Preventive BASILICA achieves ~93% procedural success to prevent coronary obstruction during high-risk TAVR.
Why the study?
Coronary obstruction is an infrequent but devastating complication of TAVR with a mortality rate approaching 50% and a 3- to 4-fold higher incidence in valve-in-valve procedures. Both the chimney snorkel and BASILICA preventive techniques lacked long-term prospective outcomes and head-to-head comparison data.

Dhamija et al. (2019) conducted an editorial in Delayed coronary obstruction after transcatheter aortic valve replacement (TAVR). Transcatheter aortic valve replacement (TAVR) with coronary protection techniques (chimney snorkel stenting or BASILICA) was evaluated. Coronary obstruction occurs in 0.35% to 4.0% of TAVR procedures with nearly 50% mortality, and preventive BASILICA achieved 93% procedural success with 0% obstruction at 30 days in 30 patients.
