Abnormal radionuclide images and minor vascular abnormalities seen on carotid angiograms indicated the site of cerebral damage resulting from herpes simplex encephalitis in 2 patients. The temporal lobe mass effect has been stressed in previous reports; however, in these cases the diagnosis was considered, biopsy undertaken, and treatment initiated before a mass or displacement of midline .structures appeared. The increasing availability of antiviral agents and their potential in the treatment of this devastating disease make the recognition of early changes important.
