Sequential treprostinil and axial-flow MCS reduces PVR ~84%, enabling heart-kidney transplantation.
Case Report (n=1)
Sequential use of intravenous treprostinil and temporary axial-flow mechanical circulatory support can successfully reduce pulmonary vascular resistance in patients with D-TGA and a failing systemic right ventricle to allow for orthotopic heart transplantation.

Bravo‐Jaimes et al. (2022) conducted a case report in D-transposition of the great arteries with advanced heart and kidney failure (n=1). Temporary axial-flow mechanical circulatory support (Impella 5.5) and intravenous treprostinil vs. Baseline hemodynamics was evaluated on Pulmonary vascular resistance (WU). Sequential use of intravenous treprostinil and a temporary axial-flow device successfully reduced pulmonary vascular resistance from 12.6 to 2 WU, enabling successful orthotopic heart and kidney transplantation.
