Chronic spontaneous urticaria (CSU) is a debilitating skin condition characterized by the presence of wheals (hives) and/or angioedema for more than 6 weeks with no specific trigger. Omalizumab, an anti-IgE monoclonal antibody, is approved for the treatment of H1-antihistamine–refractory CSU at a guideline-recommended dose of 300 mg every 4 weeks (Q4W) (also approved in the United States at 150 mg Q4W),1 independent of the total serum IgE level and body weight (https://www.gene.com/download/pdf/xolair_prescribing.pdf).
