Increasing average life expectancy has led to elderly chronic and comorbid patients requiring recurrent hospitalizations often with disability outcomes, prompting the need for an innovative hospitalization model to promote patient independence.
An innovative clinical and management hospitalization model based on interdisciplinary care pathways and digitalization is proposed to address the needs of elderly, comorbid patients.
