Robotic adrenalectomy and venous thrombectomy yields zero recurrence at one year in metastatic ESC-RCC.
Case Report (n=1)
No
This case demonstrates that ESC-RCC can produce late adrenal metastases with venous thrombus, and that surgical resection can achieve favorable outcomes at 1 year.
Single-case success with robotic adrenalectomy for ESC-RCC metastasis; hypothesis-generating and should not yet change practice.
Eosinophilic solid and cystic renal cell carcinoma (ESC-RCC) is a rare, indolent renal tumor. We report a 57-year-old female who underwent left radical nephrectomy with adrenal sparing for ESC-RCC in 2018. Six years later, she developed a solitary ipsilateral adrenal metastasis with tumor thrombus in the adrenal and residual renal veins. Pazopanib was discontinued. Robotic adrenalectomy and venous thrombectomy succeeded. Pathology confirmed metastatic ESC-RCC. At one-year follow-up, no recurrence. This case shows ESC-RCC can produce late adrenal metastases with venous thrombus, and resection achieves favorable outcomes.

Chen et al. (2026) conducted a case report in Eosinophilic solid and cystic renal cell carcinoma (ESC-RCC) with late adrenal metastasis and venous tumor thrombus (n=1). Robotic-assisted left adrenalectomy with venous thrombectomy was evaluated on Recurrence at one-year follow-up. Robotic-assisted adrenalectomy and venous thrombectomy achieved no recurrence at one year in a patient with late adrenal metastasis from ESC-RCC.
