Herpes zoster ophthalmicus (HZO) and delayed contralateral hemiplegia are a recognized association. Focal cerebral vasculitis within the sensory field of the trigeminal nerve is the likely link between these conditions. If this is the case, then a similar process should occur in other anatomic distributions. We present the first reported case of brainstem infarction following cervical herpes zoster
