Glucose infusion completely resolved repeated right hemiparesis and localized high signal intensity on DWI in the left posterior limb of the internal capsule caused by hypoglycemia.
Case Report (n=1)
This case report highlights that the posterior limb of the internal capsule is highly vulnerable to hypoglycemia, which can cause recurrent stroke-like hemiparesis that rapidly resolves with glucose administration.
Recurrent hemiparesis with focal internal capsule DWI changes may reflect hypoglycemia; this single case leaves open whether glucose testing alters stroke-mimic management.

Kubota et al. (2014) conducted a case report in Hypoglycemic hemiparesis (n=1). Glucose infusion was evaluated on Recovery of neurological symptoms. Glucose infusion completely resolved repeated right hemiparesis and localized high signal intensity on DWI in the left posterior limb of the internal capsule caused by hypoglycemia.
