To quantitatively evaluate left ventricular systolic ejection dynamics across various cardiac disease states using cineangiographic volume and shortening velocity indices.
Analyzed left ventricular cineangiograms in 48 patients across five diagnostic cohorts: controls (n=9), congestive cardiomyopathy (n=5), hypertrophic cardiomyopathy (n=9), ischemic heart disease (n=9), and mitral stenosis (n=16).
Calculated early-, mid-, and late-systolic volume change rate (deltaV/deltat), fractional stroke volume (deltaV/SV), normalized systolic ejection rate (NSER), and circumferential fiber shortening velocity (Vcf), correlating them with standard ejection and isometric phase parameters.
Patients with congestive cardiomyopathy and ischemic heart disease showed lower deltaV/deltat, deltaV/SV, NSER, and Vcf during early systole and higher deltaV/SV during mid-systole compared with controls.
Early-systolic NSER and Vcf were significantly depressed even in ischemic heart disease patients exhibiting normal global ejection fraction and no wall motion asynergy, demonstrating strong correlations with max dp/dt and ejection fraction.
