AI-enhanced telemedicine is linked to ~40% lower 30-day readmissions versus conventional telemedicine in chronic disease.
Why the study?
Chronic diseases drive hospital readmissions, and conventional telemedicine supports continuity of care but is limited by its reactive design.
Does AI-enhanced telemedicine reduce hospital readmissions and improve outcomes in patients with chronic diseases compared to conventional telemedicine?
Does AI-enhanced telemedicine reduce hospital readmissions and improve outcomes in patients with chronic diseases compared to conventional telemedicine?
AI-driven telemedicine significantly reduces hospital readmissions and healthcare costs while improving patient-reported outcomes in chronic disease management.
AI-driven telemedicine may lower readmissions in chronic disease; leaves open need for RCTs before practice change.
Chronic diseases such as heart failure, diabetes, and COPD remain major drivers ofhospital readmissions, adding to healthcare costs and reducing quality of life. Conventionaltelemedicine supports continuity of care but is limited by its reactive design. This study assessed the impact of AI-driven telemedicine platforms on readmission rates andpatient outcomes using a multicentre, quasi-experimental design involving 3,200 patients in theUK, Canada, and India. Patients were enrolled in either AI-enhanced telemedicine or conventionaltelemedicine programmes. The AI platforms integrated predictive analytics, wearable monitoring, and automated reminders to provide continuous risk assessment and early alerts.Findings showed lower readmission rates in the AI group compared with controls (30-day: 9.7%vs. 16.3%; 90-day: 18.1% vs. 27.4%). Patients in the AI group also reported higher medicationadherence, greater satisfaction, and improved quality of life. Economic analysis revealed a 34%reduction in per-patient healthcare costs, primarily due to fewer readmissions and emergencyvisits.Overall, AI-driven telemedicine offers a scalable, cost-effective, and patient-centred approach tochronic disease management, supporting its integration into long-term health strategies.

Jahani et al. (2026) studied Chronic diseases (heart failure, diabetes, COPD) (n=3,200). AI-enhanced telemedicine vs. Conventional telemedicine was evaluated on 30-day readmission. AI-enhanced telemedicine reduced 30-day (9.7% vs 16.3%) and 90-day (18.1% vs 27.4%) hospital readmission rates compared to conventional telemedicine in patients with chronic diseases.
