A 53-year-old man with alcoholic liver disease was referred for a Tc-99m sulfur colloid liver-spleen imaging. Incidental discovery of photopenic defects in the bone marrow prompted further evaluation with bone imaging and lead to the diagnosis of metastatic prostate carcinoma. In liver-spleen imaging, shift of radiocolloid uptake to the skeletal (bone marrow) reticuloendothelial system commonly occurs in patients with hepatic cirrhosis and may occur in other extensive liver disease. This colloid shift can facilitate the detection of skeletal lesions that replace viable bone marrow.
