Is preoperative PTCA and specific perioperative management safe and effective in preventing cardiac complications in patients with CAD undergoing gastroenterological surgery?
Preoperative PTCA and careful perioperative management can safely prevent critical cardiac complications in patients with severe CAD undergoing gastroenterological surgery.
May support preoperative PTCA in CAD patients for gastroenterological surgery; hypothesis-generating and should not yet change practice.
We studied 70 patients with coronary artery disease (CAD) who underwent a gastroenterological surgry in respect of the evaluation of the cardiac funcion, perioperative management and treatment for critical CAD. Forty-four patients had suffered from previous myocardial infarction (MI). Most patients had good left ventricular function. The GO-NLA with epidural anesthesia and administration of nitroglycerine was useful to manage these patients. Nine of 36 patiens who underwent coronary angiography had extensive stenosis of the coronary artery and ischemic change. Percutaneous tansluminal coronary angioplasty (PTCA) was performed before surgery in eight patients and concomitant coronay artery bypass grafting in one. The duration between PTCA and surgery was 3 to 57 day. We took gastroenterological surgery safely without any critical cardiac complications including MI in all patients. It is thought that preoperative PTCA is useful to treat patients with critical coronary artery stenosis scheduled for a gastroenterological operation.
