Preoperative cardiovascular evaluation and tailored pharmacological management are essential to reduce perioperative complications in patients with cardiovascular disease undergoing non-cardiac surgery.
This review outlines a step-by-step algorithm for preoperative cardiovascular evaluation and pharmacological management of patients with cardiovascular disease undergoing noncardiac surgery.
Due to the tendency towards significant extension of cardiovascular diseases and increase in number of patients with this pathology, especially among the elderly, there are a growing number of patients who need noncardiac surgery. In the article the question of management of patients with cardiovascular disease during their preparation for surgery is considered, as well as during pre- and postoperative periods. The main task of the doctor in preparation of the patient for surgery is preoperative cardiovascular evaluation: determination of the type of surgical intervention required, its volume, risk of complications, monitoring of cardiac events, a comparison of thrombotic and bleeding risks, the ability to continue receiving cardiac medications. We provide step-by-step algorithm of preoperative examination in patients. The pharmacological approaches for patients who are to be operated are considered in detail.
