Vasopeptidase inhibitors represent a novel therapeutic concept for hypertension and heart failure by simultaneously blocking the renin-angiotensin system and preventing the degradation of natriuretic peptides.
Should not yet change practice in hypertension or heart failure; leaves open the promise of vasopeptidase inhibitors for randomized confirmation.
Vasopeptidase inhibitors: a novel therapeutic strategy in the treatment of hypertension or heart failure The circulation is regulated by haemodynamic, structural and neurohumoral factors. Despite the importance of neurohumoral systems for the maintenance of vascular homoeostasis, their activation in disease—e.g., in heart failure—can be hazardous. The obvious clinical benefit of blocking the renin-angiotensin-system (RAS) leads to the development of new drugs which additionally block other humoral systems. The neutral endopeptidase (NEP), a membrane-bound zink-metalloprotease with structure and catalytic function similar to the angiotensin-converting enzyme (ACE), is a key-enzyme in the degradation of natriuretic peptides. Since natriuretic peptides can be regarded as endogenous inhibitors of the RAS, the combined inhibition of ACE and NEP by so called vasopeptidase inhibitors appears to be a promising strategy in the treatment of hypertension, heart failure, and potentially other cardiovascular diseases.
