CMR reveals transmural infarction with microvascular obstruction in a hormone-treated transgender woman with Takotsubo-like syndrome.
Why the study?
Although Takotsubo-like cardiomyopathies related to microvascular dysfunction are well described in cisgender women, their presentation in gender-diverse populations is poorly reported.
Case Report (n=1)
This case highlights that Takotsubo-like syndrome can be caused by myocardial infarction due to microvascular obstruction, emphasizing the importance of multimodal imaging in gender-diverse populations on hormone therapy.
CMR differentiates infarction from Takotsubo mimics; hypothesis-generating for microvascular obstruction mechanisms.
CASE SUMMARY: The patient was referred for coronary angiography within 24 hours of symptom onset, showing no epicardial obstruction and apical akinesia on ventriculography, suggestive of Takotsubo syndrome. Cardiac magnetic resonance imaging ultimately revealed transmural infarction with extensive microvascular obstruction. DISCUSSION: Microvascular dysfunction can underlie Takotsubo-like cardiomyopathies, particularly in acute, prothrombotic states. Although well described in cisgender women, its presentation in gender-diverse populations is poorly reported. TAKE-HOME MESSAGES: Takotsubo-like syndrome may present with life-threatening arrhythmias and its main cause is myocardial infarction, especially due to microvascular obstruction. This case illustrates a severe myocardial infarction with nonobstructive coronary arteries-related Takotsubo-like syndrome, highlighting diagnostic challenges and the importance of multimodal imaging.

Silva et al. (2026) conducted a case report in Takotsubo-like syndrome / Myocardial infarction with nonobstructive coronary arteries (n=1). Gender-affirming hormone therapy was evaluated. Cardiac magnetic resonance imaging revealed transmural infarction with extensive microvascular obstruction in a transgender woman presenting with Takotsubo-like syndrome under hormone therapy.
