A seriously ill, elderly patient with fever and extensive pulmonary infiltrate improved dramatically after treatment with gentamicin. After completion of therapy, a diagnosis of tularemia was established serologically. This case is reported to document the efficacy of gentamicin in the treatment of tularemia and to illustrate several difficulties in making the diagnosis of this disease.
