Intravenous thrombolysis with rt-PA was safely administered and improved neurological deficits in a patient with acute ischemic stroke 18 hours after the last dose of rivaroxaban.
Case Report (n=1)
No
Intravenous rt-PA can be safely administered in acute stroke patients taking rivaroxaban if sufficient time (e.g., 18 hours) has passed since the last dose to allow for diminished anticoagulant effect.
Case report of safe rt-PA use 18 h post-rivaroxaban does not support practice change; leaves open need for prospective safety data.

Watanabe et al. (2014) conducted a case report in Cardiogenic cerebral embolism (n=1). rt-PA was evaluated on Clinical improvement and safety. Intravenous thrombolysis with rt-PA was safely administered and improved neurological deficits in a patient with acute ischemic stroke 18 hours after the last dose of rivaroxaban.
